How to Redeem Your MyHealth Benefits
Once you have joined MyHealth, you will receive a set of unique reference codes corresponding to each of your six GP consultations and your general health check appointment included in the membership.
Receiving Your Unique Codes
You can obtain your unique reference codes in one of the following ways:
- By phone: Call our team on 01323 402790
- In person: Visit the Clinical Suite reception desk at SDHC
Booking GP Appointments & Health Checks
To redeem your GP appointments or general health check, please contact us by phone or visit us in person, providing your full name along with the unique reference code for the specific appointment you wish to book. Our team will verify your membership and confirm your booking accordingly.
Sharing Appointments with Family or Friends
If you wish to allow a friend or family member to use one of your 6 GP appointments or your general health check, please ensure the following:
- Provide them with the unique reference code associated with the appointment
- They must contact us directly via phone or in person
- They must quote your full name and the reference code when booking (our team will then collect their details to confirm the appointment)
